Centric Care Management, led by owner and founder Jennifer, focuses on advancing chronic care management and supporting medical practices to improve patient outcomes and increase revenue. With 28 years of nursing and care management experience, Jennifer is a distinguished leader adept at navigating the complexities of the healthcare system to ensure clients receive optimal care and achieve their highest quality of life.

Jennifer's expertise includes chronic disease management, population health, and transitions of care. She holds a Bachelor's degree in Nursing from George Mason University and a Master's Degree in Healthcare Administration from Grand Canyon University. As an ACM accredited care manager through the American Care Management Association (ACMA), she has presented nationally on best practices in care management and published in peer-reviewed literature.

Jennifer provides comprehensive chronic care management and supports value-based care groups, practices, and ACOs in managing high-utilizer populations. Her extensive knowledge covers Medicare/Medicaid, post-acute care, long-term care, and hospice services.

Understanding the critical role of case management in addressing social determinants of health, Jennifer leads her team to not only identify SDOH but provide solutions. This approach enhances patient health and well-being while aligning with financial stewardship goals, promoting sustainable, high-quality care.
