We will schedule a visit to ensure that your transition from hospital to home is safe and identify any further care you may need. How do I know when its time for professional help?
Often we receive requests for care management services when there is an event such as an unexpected hospitalization or in a crisis situation such as a fall at home. However, we recommend that case management services be initiated early on to keep you or your loved one safe and prevent such adverse events from occurring. If you are noticing a functional or cognitive decline, increasing healthcare needs or cost of care is becoming unmanageable then it is time to call Centric Care Management. We will discuss how our team can help you and your family put together an individualized plan of care to help you problem solve any issues at-present and prevent any issues in the future. In addition, we will coordinate an Advanced Care Plan specific to your vision of quality care and wishes when life nears the end.
